Evaluation: Medtronic Kendall SCD 700 Intermittent Pneumatic Compression System for DVT Prevention

  • The primary purpose of the Kendall SCD 700 system is to help prevent the development of DVT by promoting the circulation of blood in the legs.
  • The Kendall SCD 700 system is not indicated for any supplementary uses when used with its leg garments. However, when used with foot garments, the system is also indicated for reducing edema (acute and chronic), healing leg ulcers, preventing venous stasis, managing pain due to trauma, and enhancing circulation.
  • Kendall SCD 700 series controller
    • The SCD 700 series controller is the pump that inflates the bladders within the attached garment(s). The pump adjusts the pressure in the attached garments throughout the compression cycles.
    • The controller has a battery-status LED display on the face itself (not on the display screen). The display consists of a battery icon with three bars to indicate battery life, plus an LED icon to indicate charging. Each bar in the battery icon represents about a third of the total battery life. When less than 34% battery charge remains, only one of the bars will be lit (green). As the battery depletes further, this bar will change to amber, and then to red when there is less than 15 minutes remaining.
    • The controller has four physical buttons—the power button, an arrow button, an "A" button, and a "B" button.
      • The power button turns on the system. A green LED shows that the controller is on. Powering on the system will automatically begin a pressure cycle.
      • The arrow button is used to access the "Compliance Meter" screen further explained below.
      • The "A" and "B" buttons are used to confirm settings on the system or used to navigate the "Compliance Meter" screen.
    • Vascular Refill Detection
      • Medtronic's Vascular Refill Detection is an automatic feature that adjusts cycle time based on the time it takes for a patient's veins to refill after compression.
      • This is done after the system reaches its target pressure and then again every 30 minutes. The cycle time may range from 20 to 60 seconds for a single garment.
      • When two garments are in use, the longer refill time will be used for both garments.
    • Controller dimensions are 196 × 173 × 185 mm (7.7 × 6.8 × 7.3 in). The specified weight is 2.3 kg (5.0 lb).
  • Color LCD display—The controller display has two main screens:
    • The screen that is displayed automatically, which is divided in half (sides A and B) with one leg displayed on each side. When one or more garments are connected to the A and/or B ports on the back of the controller, a garment icon will appear on the corresponding leg(s) on the display, either as a foot cuff or as a leg sleeve garment depending on the type of garment in use. There is only one leg garment icon for all lengths of leg garments.
    • The "Compliance Meter" screen, which displays how long the pump has been running as an indication of the length of time a patient has been receiving treatment. This can be accessed with the physical arrow button on the controller.
      • Within the "Compliance Meter" screen, users can reset the meter for a new patient or change the time over which compliance is being assessed.
      • The meter displays two numbers, one on top of the other, showing the cumulative time that the system has been in operation (top number) over a selected time period (bottom number). This tells the clinician, for example, that the patient has been receiving therapy for 17 hours out of the past 24 hours. The bottom number defaults to the time since the meter was last reset, which should be indicative of the time since a patient began receiving therapy. However, users can also set this to 8, 10, 12, or 24 hours to see the amount of therapy a patient has received in the previous shift, day, etc.
    • Alarms or alerts that come up often take over the entire display, especially when they result in a stop in treatment.
  • Garments

    • Garments for DVT prevention wrap around a part of the patient's anatomy and inflate/deflate in cycles to promote blood flow. The Kendall SCD Comfort garments and Express (economy) garments are all intended for single use.
    • The SCD 700 system is available with garments that cover three different application sites—the feet, the calf (called Knee Length by Medtronic), and full leg (called Thigh Length by Medtronic). The calf- and full-leg-style garments, referred to collectively as leg garments hereafter, are available in a single inflation style that Medtronic calls "sequential."
      • Based on ECRI Institute's classification scheme, all of Medtronic's leg garments would be categorized as gradient, sequential. (That is, bladders inflate in sequence beginning with the most distal and ending with the most proximal, and the highest pressure is found at the distal end of the garment and the lowest pressure at the proximal end.       • The table below lists the garments available with the SCD 700 system. Note that our testing was done only on the Kendall SCD Comfort leg sleeve garments and the Express foot cuff garments (not the Express leg sleeve garments).
