A risk manager recently asked about implementing a new process in their facility for patients to complete the Protocol for Responding to and Assessing Patients' Assets, Risks, and Experiences (PRAPARE) and Patient Health Questionnaire Quick Depression Assessment (PHQ-9) forms via the facility's patient portal.
