Reduce Risk of Errors during Transitions in Ambulatory Settings with New Resource from AHRQ

​A new resource from the Agency for Healthcare Research and Quality (AHRQ) aims to reduce risk of patient safety errors occurring during transitions in ambulatory settings. Patients are particularly vulnerable to patient safety errors while transferring from one ambulatory-facility clinician to another, AHRQ said, in part due to a lack of effective communication and lack of patient engagement.
