In an April 10, 2015, Urgent Medical Device Recall Notification letter submitted by an ECRI Institute member hospital, Teleflex states that if the integrated circuit in the power boards used in the manufacture of the humidifiers below fails, power may be interrupted to the unit, leading to its shutdown and a blank visual display screen, potentially causing a delay in patient treatment. Teleflex further states that it has received no reports of illnesses or injuries as a result of this problem. Teleflex states that in a majority of cases, this problem will generally be noted when the device is initially turned on and before patient use because the control panel will not illuminate and the user will not be able to input settings.
