In a March 1, 2021, Urgent Field Safety Notice letter posted by the U.K. Medicines and Healthcare Products Regulatory Agency (MHRA), Leckey states that it has received reports that the boss may become loose from the tube on the above frames, causing the castor to loosen from the rear leg of the chassis (see Figure 1 in the letter). This problem is the result of an insufficient weld between the leg and the boss in some cases.
