“Passports” Help Patients Communicate Key Information to Healthcare Providers

Patient passports are some hospitals' answer to the concern that patients feel unheard by their doctor or like they are not a participant in their care. According to a recent article in the Wall Street Journal, patient passports include basic medical information (e.g., diagnosis, prescription medications) as well as patient-provided information, such as activities they need help with, concerns, goals, coping strategies, and more.
