Prism—C300/P300 Series Ceiling Lifts: Drive Gear Shaft May Fail and/or Free Fall Protection System May Not Engage Property

In a September 8, 2015, Field Safety Notice letter submitted by an ECRI Institute member hospital, Prism states it has received 17 reports of incidents in which the drive gear shaft in the above lifts broke, eliminating the lift's ability to raise or lower a patient. In the 17 incidents, 3 included reports of the free fall protection system not engaging properly. If both problems occur simultaneously, the lift may unexpectedly drop the occupant.
