  • The Vivify Health Pathways Home solution is intended to enable remote patient monitoring at home. Clinician-end analysis software—Pathways—is used to analyze patient data and identify potential deterioration in the patient's condition. The system is primarily used to manage patient populations with chronic diseases to reduce hospital readmissions. It can also be configured for use cases beyond chronic disease management, including, for example, postsurgical monitoring.
  • Major system components and software features: a) Remote patient monitoring kit—A patient-specific kit containing devices associated with the management of the patient's condition. The kit includes the following components:
    • Pathways Home—Patient-end software and tablet
      • Central hub for collecting information from accessory vital signs measurement devices and other patient-entered information via a Samsung tablet.
      • The tablet has wireless connections to the accessory devices. It also supports the use of video calls between the patient and the clinician, or with multiple participants such as family members. Vivify provides configurable video- and text-based educational content. The tablet can operate on cellular communications or through a Wi-Fi network.
    • Accessory vital signs measurement devices—Used to gather physiologic measurements pertaining to the patient's condition. The following devices can connect wirelessly:
      • Weight scale
      • Pulse oximeter
      • Blood pressure meter
      • Blood glucose meter (an accessory adapter is required)
      • Thermometer
      • Spirometer
