Online Dashboard for Communicating Patient Information in HCBS

​In partnership with a local home care agency, researchers have developed a working prototype called the Home Care Continuation Dashboard to improve communication of information on patients receiving home- and community-based services (HCBS), reports an article in the December 2012 Annals of Long-Term Care. The team explored health information technology (IT) as a way to improve the exchange of health information (especially between home care case managers and physicians), care coordination, access to services, chronic care, and prevention. Most existing health IT platforms are designed to serve as electronic health records (EHRs) in hospitals or physician offices; no health IT applications exist to address the medically complex needs of older adults receiving HCBS.
