​Root cause analysis (RCA) is a process used in many organizations to delve deeper into events. It may help provide structure for the investigation, and it typically involves mapping of events, causal factors, and root causes. The process aims to identify deeper organizational issues (systems issues) that underlie events and address them in an attempt to prevent future events.
