​A report released November 13, 2014, by the Institute of Medicine (IOM) recommends 12 measures addressing social and behavioral factors for inclusion in all electronic health records (EHRs). An IOM committee conducted a two-phase study. During the first phase, the committee identified 17 social and behavioral domains that were most strongly associated with health; this phase was discussed in an April 8, 2014, report. During the second phase, which is discussed in the more recent report, the committee identified 12 measures related to 11 of the original domains and examined the potential for their incorporation into EHRs.
