Report: PCMH Achieves Significant Cost and Quality Gains in Public, Private Demonstrations

A report released by the Patient-Centered Primary Care Collaborative summarizes the gains in care quality and cost savings achieved by organizations that have adopted the patient-centered medical home (PCMH) model of care delivery. The report provides outcomes from PCMH initiatives in both the public and private sectors that demonstrate the model's effectiveness at improving health outcomes and care quality while controlling costs. According to the report, the adoption of the PCMH model by physician practices, healthcare facilities, insurance plans, employers, and state and federal programs is expected to continue and will evolve to better connect with the medical community through accountable care organizations and other integrated systems of care.
