​Patient safety in ambulatory care settings has been "largely ignored" since the Institute of Medicine (now the National Academies of Science, Engineering and Medicine) called attention to medical errors in its 1999 report, To Err Is Human: Building a Safer Health System. So says the American College of Physicians (ACP) in a new policy paper setting forth recommendations for improving patient safety in ambulatory care, especially office-based practices.
