The Joint Commission provides sentinel event statistics in a September 30, 2014, update of its Sentinel Event Data Summary web page. Of the 394 events reported from January to June 2014, unintended retention of a foreign body (n = 57); fall (n = 44); suicide (n = 39); wrong-patient, wrong-site, or wrong-procedure surgery (n = 35); and delay in treatment (n = 34) were among the most frequently reported.
