ISMP Releases High-Alert Medication Self-Assessment Tool

​A tool released on September 21, 2017, by the Institute for Safe Medication Practices (ISMP) aims to help organizations asses the safety of their systems and practices for high-alert medications and to identify opportunities for improvement. High-alert medications are those that bear an increased risk of causing significant harm if they are used in error, ISMP said. Since not all targeted high-alert medications will be used in every facility, the tool allows users to choose one or more high-alert medications to focus the assessment on, ISMP said.
