A pharmacist-led medication review is an effective method to reduce the incidence of drug-related problems among elderly patients discharged from the hospital, concludes a research letter published in the September 24, 2012, Archives of Internal Medicine. The letter details the results of a study involving 340 patients who were 60 years or older and using at least five prescribed drugs at the time of discharge from the hospital.
