ISMP Releases Tubing Misconnections Self-Assessment Tool

The Institute for Safe Medication Practices (ISMP) has collaborated with Baxter Healthcare’s Clinical Center of Excellence to develop a Tubing Misconnections Self Assessment for Healthcare Facilities. While new international standards are being developed to address this issue, hospitals can complete the assessment to help better identify products and practices that pose a risk of inadvertent tubing misconnections and mitigate potentially known risks.
