Leadership Engagement in Event Review Spurs Process Improvements for MA Hospital

A Massachusetts hospital engages its leaders in the organization’s patient safety issues by providing two to four selected event reports for review at a weekly meeting of the hospital’s service operations committee, whose members include senior executives. The community teaching hospital’s approach to organizational patient safety and a just culture is described in an article in the May-June 2012 issue of Patient Safety and Quality Healthcare by the hospital’s directors of risk management and quality. The committee—comprising executive management, service chiefs, and service line managers—reviews the reports to identify opportunities for organizational learning. The reports are selected for the committee’s review by the hospital’s healthcare quality team, whose members include professionals in risk management, quality improvement, and patient safety.
