A Massachusetts hospital engages its leaders in the organization’s patient safety issues by providing two to four selected event reports for review at a weekly meeting of the hospital’s service operations committee, whose members include senior executives. The community teaching hospital’s approach to organizational patient safety and a just culture is described in an article in the May-June 2012 issue of Patient Safety and Quality Healthcare by the hospital’s directors of risk management and quality.
