IHI Updates Guides for Improving Care Transitions

The Institute for Healthcare Improvement (IHI) has updated its guides for improving care transitions from the hospital, a postacute care setting, or a rehabilitation facility to other care settings in an effort to reduce avoidable rehospitalizations. Each of the four guides addresses transitions from the hospital to skilled nursing facilities, community settings, home healthcare, and clinical office practices, respectively (free registration required). For example, the guide on improving transitions from hospitals to skilled nursing facilities lists three key changes that are necessary for improvement. The first is to ensure that the facility is ready and able to care for the individual.
