The Joint Commission identifies its five most challenging requirements for 2011 across each of its accreditation programs in the April 2012 Joint Commission Perspectives. The two most problematic standards for hospitals were related to maintaining complete and accurate medical records for each individual patient (RC.01.01.01) and maintaining the integrity of the means of egress (LS.02.01.20), with rates of noncompliance at 66% and 56%, respectively.
