ISMP Self-Assessment Tool Identifies Tubing Misconnection Risks

The Institute for Safe Medication Practices (ISMP) has released a new self-assessment tool for healthcare facilities that addresses catheter and tubing misconnections. Developed as part of a collaboration with Baxter Healthcare’s Clinical Center of Excellence, Tubing Misconnections Self Assessment for Healthcare Facilities guides users through a modified risk assessment. To complete the analysis, users evaluate current delivery systems and mating devices, rate the ease of connection and potential for patient harm, and assign risk priority scores.
