Falls and unintended retention of a foreign body were the two most frequently reported sentinel events in 2018, according to a March 13, 2019, alert from Joint Commission. Joint Commission reviewed 801 sentinel events in 2018, the majority (87%) of which were self-reported. A total of 111 reports each were related to falls and unintended retention of a foreign body.
