Hospital Transitional Care Unit Reduces Falls with Prevention Strategies

A transitional care unit at an acute care facility reduced its fall rate by 57% from 9.32 to 4.02 falls per 1,000 patient-days by identifying fall risks on the unit and implementing a unit-specific fall-reduction program. Many of the strategies could be applied to any acute or long term care setting that serves a geriatric population, which is most at risk of falls, according to the authors of a report on the unit's initiatives.
