Recognizing that implementing patient safety and quality initiatives can be a difficult task, Healthcare Risk Control(HRC) asked our members to share their experiences regarding barriers to achieving safer care. While many respondents contend that they have not found the proverbial silver bullet to eliminate these barriers, the responses show how much work is being done to transform healthcare.

In many of the comments left by respondents to the HRCsurvey, "journey" was a recurring theme. Overcoming these barriers truly represents the journey that each facility is on to create a culture of safety, which is "slow," possibly taking 10 to 15 years (Sorra et al.; Chassin and Loeb).
