A nurse making rounds on the night shift noticed that a blanket had begun to slip off a sleeping patient, leaving several inches of material trailing on the floor. Recognizing a slip-and-fall hazard, the nurse paused to adjust the blanket—not because an explicit rule was in place that required staff to pick up blankets, but because the perceived threat to safety prompted her to correct the problem.
