Equipment Failures to Blame for Many OR Errors

A substantial proportion of errors and adverse events that occur in ORs are caused by equipment-related failures, and procedures that rely more heavily on equipment may demonstrate a higher proportion of such failures, suggest the findings of a study published in the September issue of BMJ Quality and Safety.
