This technology assessment was commissioned by the Washington State Health Technology Assessment Program for use by the Health Technological Clinical Committee (HTCC). The HTCC uses evidence, primarily as assessed in this report, to determine whether health technologies are safe, effective, and cost effective, and therefore should be covered by state programs that pay for health care.
