As risk managers have long understood, medication errors can occur for many reasons. For example, the wrong dose or wrong form of a drug might be ordered, the drug might be administered to the wrong patient, a prescribed drug might be mistaken for another with a similar name or a similar label, or medications might be ordered and administered based on erroneous laboratory or other diagnostic data.
