Toolkit Helps Nursing Homes Evaluate Person-Centered Care

​In a pilot evaluation of a toolkit to evaluate a nursing home's progress toward person-centered care involving 12 early adopter nursing homes, project coordinators at all the nursing homes said they would recommend the toolkit to other nursing homes, reports a study in the September 2014 Journal of the American Medical Directors Association. The toolkit is available for free through the Advancing Excellence in America's Nursing Homes campaign. The toolkit has two primary components. The first component is a set of materials for interviewing residents regarding their preferences for 16 activities and personal care services, based largely on information already collected as part of the Minimum Data Set, and for following up with residents regarding how satisfied they are with how their preferences are addressed.
