AHRQ Guide on Medicaid Readmission Reduction Offers Strategies, Tools, More

​The Agency for Healthcare Research and Quality (AHRQ) has issued a guide to help acute care hospitals improve their efforts to reduce Medicaid readmissions. Commissioned to identify ways that evidence-based strategies to reduce readmissions could be adapted or expanded to better address the transitional care needs of the adult Medicaid population, Hospital Guide to Reducing Medicaid Readmissions is aimed toward hospitals at all stages of readmissions work, whether they have been focusing on Medicare patients for years, have successfully implemented best practices for all patients in all services, or are only beginning readmission reduction efforts.
