​​Implementation of a structured communication protocol for nursing home residents being transferred to the emergency department (ED) was linked to a significant reduction in unplanned 30-day hospital readmissions, according to a study published online July 20, 2015, in Annals of Long-Term Care. The study was implemented over four months at a 137-bed skilled nursing facility and included all registered nurses, licensed practical nurses, and physicians.
