ONC Posts Webpage for Health IT in Long-Term and Post-Acute Care

​The Office of the National Coordinator for Health Information Technology (ONC) has launched a new webpage focused on supporting health information technology (IT) use in long-term and post-acute care, particularly during transitions in care. The page focuses on two areas: first, emphasizing the importance of health IT during transitions in care, and second, providing examples of successful or innovative health IT implementation in long-term and post-acute settings.
