Medication Reconciliation: Using Health IT; Resolving Discrepancies

​Medication reconciliation is a “complex, human workflow process” that can be supported but not replaced by health information technology (IT), according to an article in the June 2013 Journal of Nursing Administration. The authors describe the role of health IT in supporting this process, noting that it works best when workflows are developed with the health IT in mind rather than when health IT is forced into an existing workflow. Staff input is also key.
