AHA Report Outlines Five “Guiding Principles” for Hospital Discharge Planning Tools

​The American Hospital Association (AHA) has released a report, Private-Sector Hospital Discharge Tools, describing five organizations' discharge planning tools and the lessons learned from using the tools. Depending on the example, the tools can be used to assist with postacute care placement, reduce readmissions, and manage patient transitions from acute to postacute settings.
