The winning submission describes Penn Medicine Center for Health Care Innovation’s success in developing a tool that proactively extracts information from their Electronic Health Record (EHR) to identify patients who require intervention. Some examples of patient intervention successfully identified through this application include patients who have critical medication expiring, require screening for extubation, or assessment to pinpoint the cause behind excessive emergency room visits.

Penn’s app notifies clinicians promptly about the need for intervention through a secure text or email. The alerts are also displayed through the system’s alert dashboard. The team at Penn Medicine frequently identify new patient interventions, which could benefit from this application.
