CME Accreditation Statement:
This live activity has been planned and implemented in accordance with the Essential Areas and Policies of the Accreditation Council for Continuing Medical Education (ACCME). ECRI Institute is accredited by the ACCME to provide continuing medical education for physicians.
 
AMA Credit Designation Statement:
ECRI Institute designates this live activity for a maximum of 10 AMA PRA Category 1 Credit(s)tm. Physicians should claim only the credit commensurate with the extent of their participation in the activity.
 
Note: November 13th is worth 6.25 AMA PRA Category 1 credits; November 14th is worth 3.75 AMA PRA Category 1 credits.

California Nursing Contact Hours:
This activity has been approved for a total of 12 California State Nursing contact hours by the provider, Debora Simmons, who is approved by the California Board of Registered Nursing, Provider Number CEP 13677. 
(7.5 for Day 1 and 4.5 for Day 2).

You must sign in and out each day you are requesting credit on the sign-in/out sheet located at the registration desk. Submit your completed forms on your final day of attendance at the conference to the registration desk. Pennsylvania CLE Credits:
This program has been approved by the Supreme Court of Pennsylvania’s Continuing Legal Education Board for up to a total of 10.5 credits: 1 ethics* and 9.5 substantive Pennsylvania CLE credits.

You must sign in and out each day you are requesting credit on the sign-in/out sheet located at the registration desk. At the registration desk, please pick up the “CEU Packet,” which contains a Pennsylvania CLE form entitled “Credit Request Form” that must be returned with payment DIRECTLY to the PACLE Board.

*You must attend both days in order to qualify for the ethics credit.
