EMPLOYEE BENEFIT PROGRAM

Effective 03/01/08

Health Insurance. Coverage is available through Aetna for employees and their dependents from the first day of employment. Program choices are Aetna HMO, Aetna Quality Point of Service (QPOS) and Aetna Choice POS. Employee contributions vary depending on choice of plan and type of coverage. See the Employee Contributions sheet for exact amounts. If your spouse is eligible for health insurance elsewhere but chooses ECRI Institute coverage, there is an additional $40-per-pay copay. Dental, prescription drugs, and vision are included in the health insurance coverage.

Life Insurance. After 90 days of employment, ECRI Institute provides group life insurance coverage for employees equal to 2½ times their annual base salary up to a maximum of $750,000 at no cost to the employee. Proof of good health may be required for amounts over $375,000.

Short-Term Disability. After 90 days of employment, ECRI Institute pays 66 2/3% of an employee’s basic weekly earnings. Weekly benefits begin on the 31st day of disability caused by sickness or accidents and continue for 22 weeks.

Long-Term Disability. After 90 days of employment, ECRI Institute plan pays 66 2/3% of basic monthly earnings up to a maximum payment of $10,000 per month. Monthly benefits begin on the 180th day of disability caused by sickness or accidents.

Holidays. ECRI Institute recognizes ten paid holidays each year, including seven designated holidays (New Year’s Day, Memorial Day, Independence Day, Labor Day, Thanksgiving, the day after Thanksgiving, and Christmas) and three personal days every anniversary year. The three personal days are available after 90 days of employment.

Vacation. Employees accrue two weeks of vacation each year. They are eligible to take vacation after six months of continuous employment. After four years, employees accrue three weeks, and after seven years, they accrue four weeks. Vacation time cannot be accumulated beyond a maximum of two years. Any unused vacation time beyond the maximum is forfeited.

Sick Leave. Fully paid sick leave is accrued at the rate of 10 days per year. Sick leave is cumulative, to a maximum of 90 days.

Retirement Savings Plan. ECRI Institute sponsors a matching 403(b) program with TIAA-CREF. After 90 days of employment, ECRI Institute will match ½ of your contribution to the 403(b) to a maximum of 3% of your salary. ECRI Institute's contribution is re-evaluated and determined on an annual basis.

Flexible Spending Accounts. These accounts are designated for medical and/or dependent care expenses. You allot a specified dollar amount to be deducted on a pre-tax basis from your pay. That money is then available to reimburse payment of the expenses mentioned above when appropriate proof of payment is submitted to the payroll department. This benefit is available after successful completion of the 90-day probationary period.

Opt Out. Cash in lieu of health insurance benefits is available to all employees who are eligible to be enrolled in ECRI Institute's health insurance program but are also able to be enrolled in health coverage through a source other than ECRI Institute. That source must provide comprehensive benefits for hospital, surgical, and other medical care. Opt-outs will receive $68.75 per pay ($1,650 per year).

The benefits listed above are subject to change. Please be sure to read the insurance booklets and the procedures manual thoroughly for more complete coverage details.
